What is Concierge Medicine?
Concierge medicine is a relationship between the patient and the primary care provider. The patient pays an annual fee for a wide range of services which includes 24/7 access to your provider via text, phone, video, or in-person (as appropriate for the situation).
Concierge Annual Membership
$3,500 Annually
Access:
- 24/7 access to your provider via text, phone, video, or in-person (as appropriate for the situation).
- Labs and ancillary services performed at a Facility and billed through your insurance or cash negotiated prices if requested.
Labs:
Complete Metabolic Panel, Complete Blood Cell Count, Thyroid Stimulating Hormone, Prostate Specific Antigen, Lipid Profile, Hemoglobin A1C, Urinalysis, Other labs, based on diagnosis
Ancillary Services:
Ultrasounds, X-Rays, MRI, CT, ECG, PFT, or other services based on diagnosis, billed through your insurance or cash negotiated prices if requested.
Miscellaneous:
One Complimentary IV Vitamin Nutrient Treatment annually

Physical Exams, Vertigo/Dizziness, Wound Care, GERD/Acid Reflux, Phlebotomy, Skin Infection, Conjunctivitis, Hypertension Management, H. Enroll
We look forward to your enrollment and can't wait to meet you! At Panzer Concierge Medicine, the relationship is important as we strive for you to reach your maximum health. For that reason, the first step is a screening interview to discuss your health history.
All new members will pay a one time enrollment fee of $100 at the first visit. Regular billing will start the month you enroll regardless of first visit date.
To prepare for your screening interview, please fill out the appointment form. Please note, more detailed intake will be provided at your screening. We will contact you after receiving your appointment form.
